1. The form must be completed using the Mozilla FireFox web browser. If you do not have FireFox click HERE to install.
2. To submit and send the form (Members Log In) you must use an email client such as "Outlook", Outlook Express" or "Windows Live Mail". If you do not have one of these installed choose one below to install before continuing.
Outlook Express should already be on your
computer if you have Windows XP or newer.
You should NOT pay for outlook express - it is free and comes with Windows.
Unless you need the full version of "Outlook" which is different and
comes with Microsoft Office.
You should try the Start Menu -> Find or Search-> "Outlook
Express"
3. Be sure to click "Send" AFTER verifying the information contained in the email you are sending is complete and correct.
